Homecare Case Manager [United Arab Emirates]


 
The Case Manager will be responsible for providing comprehensive case management services to individuals or families in need. The Case Manager will assess client needs, develop and implement a service plan, and monitor progress toward goals. The Case Manager will work collaboratively with other service providers and community resources to ensure that clients receive appropriate services and support.
Responsibilities:
  • Conduct initial client assessments to identify needs and develop a service plan
  • Provide ongoing case management services, including regular check-ins and follow-up
  • Monitor progress toward goals and adjust the service plan as needed
  • Collaborate with other service providers and community resources to coordinate services and support
  • Advocate for clients and connect them with appropriate resources and services
  • Maintain accurate and up-to-date case files and documentation
  • Provide crisis intervention and support as needed
  • Participate in team meetings and case conferences
  • Keep up-to-date with industry standards and best practices for case management

Requirements:
  • Bachelor's degree in social work, psychology, or related field
  • 4-5+ years of experience in case management or related field
  • Knowledge of social services and community resources
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Excellent organizational and time management skills
  • Strong problem-solving and critical-thinking skills
  • Ability to maintain confidentiality and professional boundaries
  • Experience working with diverse populations and cultural sensitivity
  • Valid driver's license and reliable transportation
