The Case Manager will be responsible for providing comprehensive case management services to individuals or families in need. The Case Manager will assess client needs, develop and implement a service plan, and monitor progress toward goals. The Case Manager will work collaboratively with other service providers and community resources to ensure that clients receive appropriate services and support.
Responsibilities:
- Conduct initial client assessments to identify needs and develop a service plan
- Provide ongoing case management services, including regular check-ins and follow-up
- Monitor progress toward goals and adjust the service plan as needed
- Collaborate with other service providers and community resources to coordinate services and support
- Advocate for clients and connect them with appropriate resources and services
- Maintain accurate and up-to-date case files and documentation
- Provide crisis intervention and support as needed
- Participate in team meetings and case conferences
- Keep up-to-date with industry standards and best practices for case management
Requirements:
- Bachelor's degree in social work, psychology, or related field
- 4-5+ years of experience in case management or related field
- Knowledge of social services and community resources
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Excellent organizational and time management skills
- Strong problem-solving and critical-thinking skills
- Ability to maintain confidentiality and professional boundaries
- Experience working with diverse populations and cultural sensitivity
- Valid driver's license and reliable transportation
